Bring original JPN letter, new IC straight to 14th floor, Menara Perkeso, Jalan Ampang (Besides Great Eastern Mall).

Driving License & Car Registration Card

1. Bring along existing Driving License & Car Registration Card.
2. Photocopy of New IC, JPN letter.
3. 1 piece of photo for Driving License
4. At JPJ, the officer will give 2 forms to fill in. Each one for Driving License (JPJ L7) & Car Registration Card (JPJ K8) respectively.

Insurance

1. Call your insurance agent to get the form.
2. Need to submit the form together with original policy, they'll update the name & return the policy.

KWSP

1. Fill in form KWSP3 (http://www.kwsp.gov.my/index.php?hdl=bin&rp=320) Permohonan Pendaftaran Ahli and Pindaan Maklumat.
2. Copy of New & Old IC.

LHDN (Income Tax Department)

1. Fill in Borang CP600B (http://www.hasil.gov.my/cP/upload/Form/Lain/CP600B.pdf)
2. Contact their Call Center (Toll Free no.: 1-300-88-3010) & Fax the borang over together with New IC.

1. Fille in Borang Permohonan JPN.KP16.
2. Old IC
3. Islamic Card

It tooks me neary 4 months to get a new MyKad. Before the card is ready, received an approval letter from JPN on the approval of name change.

*** IMPORTANT*** Keep this letter as it is needed to use to request for name change in other places.

I've converted to Muslim & need to update my Muslim name in all statutory board.
